How to Apply

Account Type Selection
Begin by accessing the signup portal. When the page loads, select "Non-Governmental Organization (NGO) (Foreign Owned)" from the account type dropdown. The detailed requirements will be displayed below the field. Click Next to continue.

Facility Information
Provide the facility details: logo, practice name, registration/CAC number, practice address, state and L.G.A of practice, contact email and phone, and the date practice commenced.

Upload Documents
Upload clear copies of the required documents: CAC Certificate (Limited Only), Memorandum and Articles of Association (MEMART), Mission and Vision statement.

Equipment & Practitioner
Declare the available equipment against the board's required equipment list and link the registered practitioner(s) responsible for the facility.

Review & Pay
Review your application, then pay the applicable fees online to submit. The board reviews submissions and schedules inspection/verification where required.
