Requirements

The registration process requires the following documentation and information:

• Facility information; name, image, address, contact details and others
• CAC Certificate (Limited Only)
• Memorandum and Articles of Association (MEMART)
• Directors; name, address and images
• Equipment & practitioner

How to Apply

   Account Type Selection

   Begin by accessing the signup portal. When the page loads, select "Optical Dispensing Lab" from the account type dropdown. The detailed requirements will be displayed below the field. Click Next to continue.

2. 2

   Facility Information

   Provide the facility details: logo, practice name, registration/CAC number, practice address, state and L.G.A of practice, contact email and phone, and the date practice commenced.

   Upload Documents

   Upload clear copies of the required documents: CAC Certificate (Limited Only), Memorandum and Articles of Association (MEMART).

4. 4

   Equipment & Practitioner

   Declare the available equipment against the board's required equipment list and link the registered practitioner(s) responsible for the facility.

5. 5

   Review & Pay

   Review your application, then pay the applicable fees online to submit. The board reviews submissions and schedules inspection/verification where required.
